CHANGES IN CARE
As a PeopleOne Health member, we want to inform you of some upcoming changes which will take effect on January 1, 2024. As you know, we are committed to delivering high quality healthcare to the businesses and community of Altoona. We will continue to do so through December 31, 2023 for all patients. As of January 1, 2024, PeopleOne Health will only provide services to those who receive coverage from their employers or have individual subscriptions.
Beginning January 1, 2024 - under the guidance of Dr. Zane Gates - Gloria Gates Care will deliver healthcare to those of you who receive coverage under Medicaid, Medicare, the Children’s Health Program (CHP), and individuals under the Affordable Care Act (ACA). Dr. Gates will continue in his current role with PeopleOne Health through the end of 2023.
Where will I go for medical care?
Dr. Gates and our team will continue to deliver care as usual through December 31, 2023 at the Altoona Main location 2525 9th Avenue Suite 2B
Where will I go for medical care after January 1st 2024?
- Medicaid, Medicare, the Children’s Health Program (CHP), and individuals under the Affordable Care Act (ACA) who select Gloria Gates Care as their primary care provider (PCP) will receive continued care at the current Altoona Main location 2525 9th Avenue Suite 2B.
- Employer-based and Individual Subscription Coverage Patients. PeopleOne Health will continue to serve membership (employer) based patients at our Altoona Intowne Square office, located at 1701 12th Ave B3.

You will need to call your insurance carrier to confirm that you want to select Gloria Gates Care as your Primary Care Provider (using the address information listed below).

Insurance companies require that patients call and choose a primary care provider. Please be sure to call your insurance company in December in order to avoid disruption in your medical care after the new year.

If you continue care with Gloria Gates Care, at your first visit you will sign a Release of Information which allows Gloria Gates Care to access your medical records.
If you choose to transfer your care to a new primary care provider in the community, please download the Medical Records Release form and submit (via fax, mail or in-person) to the respective addresses listed below.
